We are currently accepting new patients and are excited about the opportunity to serve your needs. We strive to ensure patient satisfaction with every visit. If you have a scheduled appointment as a new patient, here are some ways to prepare for your first visit:
¨ First, you'll want to transfer your medical records to us. For patient privacy reasons, we are not able to transfer records for you; only YOU can officially transfer your records. Please use this AUTHORIZATION TO DISCLOSE/TRANSFER INFORMATION form in the list of the PDF forms at the bottom. Once completed, please send it to your previous physician's office and they will transfer your records to us.
¨ Arrive for your first appointment 15 minutes early so that we will have time to process your paperwork. To minimize your stress and inconvenience, you have the option of completing and printing out various patient forms required at your first office visit; see below for details
¨ Bring a list of all of your current prescribed medications, over-the-counter medications, herbs, supplements, and vitamins with you.
¨ Be sure to bring your insurance card(s) with you on EVERY VISIT, not just your first visit.
¨ We know how annoying it can be to sit in the waiting room and have to fill out all sorts of paperwork so for you convenence, we have included all NEW PATIENT FORMS via PDF. You may complete these in the comfort of your home before your first office visit, then print them out and bring them with you to your first appointment. If you don't have a PDF reader, then follow the link below to download the program.
These are "fill-in-the-blank" forms, so all you need to do is:
1. Open the PDF. Put your curser on the first blank, then "tab" through to the next space.
2. Fill in the information. Print out the form with your completed information.
3. Don't forget to SIGN THE FORM where applicable.
4. Bring the completed form with you for your first visit.
